1. Service Description

Preferred Housing Solutions provides non-medical housing coordination services for professional referral sources, including hospitals, VA programs, behavioral health agencies, reentry organizations, and social service providers. We coordinate placement into structured housing environments for ambulatory adults capable of independent living.

2. Non-Medical Housing Coordination

PHS is a non-medical housing coordination provider. We do not provide, and these terms do not include:

  • Medical care, nursing services, or clinical treatment
  • Medication management or administration
  • Personal care assistance or mobility assistance
  • Assisted living services or skilled nursing care
  • Behavioral health treatment or monitoring
  • 24-hour supervision or locked facility care

3. Eligibility Requirements

Housing placements through PHS are available to individuals who are:

  • 100% ambulatory and capable of independent walking
  • Capable of independent activities of daily living
  • Able to self-administer medications
  • Appropriate for shared housing environments
  • Behaviorally stable and appropriate for community-based housing

4. Referral Process

Professional referral sources may submit referrals through our website or by contacting our placement coordination team. All referrals are reviewed for:

  • Housing fit and availability
  • Ambulatory status and independent living capability
  • Income stability and verification
  • Safety considerations
  • Behavioral appropriateness for structured housing

Submission of a referral does not guarantee acceptance or placement. We review each referral individually and will communicate our determination to the referring professional.

5. Housing Standards

PHS coordinates housing that meets established standards for cleanliness, safety, and maintenance. All residents must comply with housing rules and expectations, including maintaining a clean and respectful living environment.

6. Income and Payment

PHS coordinates housing for individuals with verified income sources. Agency-supported housing payments may be considered. PHS does not bill Medicaid, Medicare, or any government healthcare program for medical services as we do not provide medical care.

7. Emergency Services Disclaimer

PHS is not an emergency response provider, crisis stabilization facility, medical provider, or licensed care facility. If someone is experiencing a medical emergency, psychiatric emergency, overdose, active crisis, or immediate danger, call 911 or the appropriate crisis response provider immediately.

8. Limitation of Liability

PHS coordinates non-medical housing placements. We are not responsible for medical care, personal care, or clinical services that may be provided by third parties or at housing locations we do not own or operate.

9. Text Messaging

With your consent, we may send text messages regarding referral coordination. Standard message and data rates apply. Reply STOP to opt out. Consent is not required for referral review.
