PHS maintains structured housing coordination standards that support professional referral partners in understanding housing-fit criteria, placement requirements, and stabilization expectations.

Ambulatory Standards

All placements must meet ambulatory criteria — ambulatory adults capable of independent movement without mobility assistance.

  • 100% ambulatory capability
  • No mobility assistance required
  • Able to navigate stairs independently
  • No wheelchair or walker dependency

Independent Living Standards

All placements must meet independent living criteria — adults capable of self-care without personal care assistance.

  • ADL independence (bathing, dressing, eating)
  • No personal care assistance needed
  • Self-management of medications
  • Able to maintain personal hygiene

Transportation Considerations

Housing coordination accounts for transportation access to employment, healthcare, and essential services.
