How To Refer

  3. 3 We review eligibility during business hours

  5. 5 Coordinate move-in if placement is confirmed

Who Does NOT Qualify For Placement

PHS provides non-medical ambulatory housing only. We cannot accept referrals for individuals who:

Require nursing care or medical supervision
Need mobility assistance or wheelchair support
Require medication administration
Need personal care assistance
Require 24-hour supervision
Are actively violent or threatening
Require locked facility care
Require clinical treatment or monitoring
